Features of Gastric Surgery

“Stomach cancer” is the most popular of all the types of cancer in Japan. It arises in the cells in the mucosa of the stomach, and goes deeper invading the submucosal layer, the proper muscular layer, the subserosal layer and the serosa. In recent years, diagnostic technology has shown significant advancement, which results in a fact that more than half of the cases are detected at the early stages in which cancers are still within the mucosa or the submucosal layers. Treatment technology has also advanced lately, which has contributed to the complete cure of about 90% of the early-staged gastric cancers. Treatment method is based on the “Japanese Gastric Cancer Treatment Guidelines” developed by the Japanese Gastric Cancer Association and is chosen among endoscopic treatment, surgery and chemotherapy (therapy with anti-cancer drugs) depending on the disease progression. Generally speaking, gastric cancer can be completely resected by endoscope when it is detected at the very early stage, while surgery is the most effective treatment for the complete cure for early-staged stomach cancers, which can’t be removed by endoscope, and advanced cancers. The endoscopic resection is performed at the Division of Endoscopy, while surgeries are performed at the Division of Gastric Surgery.
Surgeries for all the tumors in stomach and surrounding regions, such as SMT (submucosal tumor) including GIST (gastrointestinal stromal tumor), which is the second-common type of tumor after gastric cancer, are performed at this division

Features and Therapeutic Strategies of the Division at the SCC

With one of the largest numbers of gastric cancer surgeries in Japan, the medical staff with ample experiences perform safe and reliable surgeries.

For an early-staged gastric cancer, a limited operation, which is minimally invasive for a patient, is the choice of procedure, while a standard gastrectomy is done for an advanced gastric cancer. These are all in accordance with the “Japanese Gastric Cancer Treatment Guidelines.” At the SCC, the number of gastric cancer surgeries is the second-largest in Japan. It proves the fact that ample experiences can guarantee an optimal choice of cancer treatment, and safe and reliable surgeries resulting in positive outcomes from the treatments.

Various surgical procedures, including laparoscopic surgery and robotic surgery, are being adopted.

Gastric cancer surgeries have variations other than a standard “open abdominal surgery,” including “laparoscopic surgery” with a tiny camera (laparoscopy) and surgical instruments being inserted into the abdomen. As laparoscopic surgery is listed as a standard procedure for an early-staged stomach cancer in the “Japanese Gastric Cancer Treatment Guidelines,” it is considered as the treatment option when the patient’s conditions and preference apply.
Laparoscopic surgery is being performed with the utmost caution at the SCC, with five surgeons with certified skills and ample experiences being in charge. Also, robotic surgery (robot-assisted gastrectomy) is being adopted proactively at the SCC. We have already performed 286 (as of the end of Dec., 2018) robotic surgeries so far. When a submucosal tumor in the stomach is smaller than 5 cm in diameter, a laparoscopic distal resection will be adopted, and when it is 5 cm or bigger, an open surgery will be performed. Also, a surgical procedure combining endoscopy and laparoscopy is being adopted in a proactive way at the SCC, as it has recently been included in the Japanese medical insurance coverage

Participation in “clinical trials” is promoted as a way of pursuing more effective cancer treatment than ever.

“Clinical trials,” which are to evaluate the effects of new treatments, play crucial roles in defining future directions of gastric cancer treatment in Japan. The Division of Gastric Surgery at the SCC is one of the major members of the JCOG (Japan Clinical Oncology Group) and is actively participating in useful clinical trials. At the same time, the Division is leading clinical trials involving other oncology groups than JCOG. Among them, a clinical trial to prevent postoperative pneumonia and delirium is being especially focused on, in the effort to ensure dependable surgeries for the aged patients

At the SCC, we perform surgeries for gastric cancers, while endoscopic resections are done at the Division of Endoscopy and chemotherapy (treatment with anti-cancer drugs) is handled at the Division of Gastrointestinal Oncology. At each division, highly advanced treatment is being provided with professional expertise, and at the same time, we all work together in tight cooperation to offer the best possible cancer treatments
