Features of Hepato-Biliary-Pancreatic Surgery

“Hepato-Biliary-Pancreatic” covers the region including liver, biliary tract (consisting of gallbladder, bile duct and duodenal papilla) and pancreas. These organs do not function as digestive tracts, which include stomach and large intestine where food is sent down for digestion, but they play vital roles assimilating and detoxifying food and nutrition, and processing waste. The Division of Hepato-Biliary-Pancreatic Surgery specializes in surgical treatments for cancers detected in this region. As the region lies deep in the abdomen and the organs there are prone to bleeding, highly advanced expertism and surgical techniques are required in order to make an accurate diagnosis and to perform a precise and safe resection of the mass.

Features and Therapeutic Strategies of the Division at the SCC

Precision of a diagnosis is ensured with examinations by the latest, state-of-art medical equipment.

As the organs treated at this division lie deep in the back, there won’t be any way to make a precise diagnosis without examinations by transverse images, like the ones obtained by CT. The SCC is equipped with the latest, state-of-art medical equipment for close examinations. We make full use of them in the efforts of providing quick and accurate diagnoses.

More than 400 surgeries are performed every year.

It is often said that you must choose a hospital with plentiful experiences of surgery when it comes to a surgical therapy for a cancer in the hepato-biliary-pancreatic region, as the organs there lie deep in the back and are prone to bleeding, which is one of the features not found in digestive tracts including stomach and large intestine. We perform more than 400 surgeries a year for cancers in the hepato-biliary-pancreatic region including nearly 200 cases of hepatectomy, more than 150 cases of pancreatic resection (i.e., pancreaticoduodenectomy, distal pancreatectomy, total resection of pancreas, etc.) and some other cases of resection in the region.

We have plentiful experiences for extremely difficult procedures.

Cancers in the hepato-biliary-pancreatic region require highly-advanced skills and experiences for resections, which will likely involve extremely difficult surgeries in addition to standard hepatectomy and pancreatectomy. For example, extended hepatectomy plus combined resection and reconstruction of portal vein and hepatic artery will need to be done for a perihilar cholangiocarcinoma, and hepatopancreas resection (hepatectomy and pancreaticoduodenectomy) will need to be done all at once for a widely spread bile duct cancer and a gallbladder cancer. We do not hesitate to tackle these difficult surgeries.

Laparoscopic surgery, which is a less-invasive procedure than a conventional laparotomy, is taken as one of surgical treatment methods.

In addition to conventional laparotomy, there is a newer type of surgery called “laparoscopic surgery,” which is to insert surgical tools into the abdomen. This is indeed a surgery far less invasive than conventional laparotomy, but it won’t apply to some difficult cases in the hepato-biliary-pancreatic region where the resections can be highly complicated. As we perform laparoscopic surgeries only for hepatectomy and “distal pancreatectomy,” which are listed as applicable cases for national health insurance coverages in Japan for ensured effectiveness and safety.

We take proactive approaches for clinical studies and trials when they seem beneficial for our patients.

We are being involved in many clinical studies and trials in pursuit of better outcomes from therapies as well as development of new cancer treatments. Among them, the JASPAC (Japan Adjuvant Study Group of Pancreatic Cancer) is a study group for adjuvant chemotherapy for pancreatic cancer and is the one we are engaged in as a leader. With this group, we announced on a study result in 2013 about dramatically improved postoperative survival rate of pancreatic cancer patients when S-1, an oral anticancer drug, was administered instead of gemcitabine, the conventional one for pancreatic cancer. The news had a huge impact and gained worldwide attention.

The best choice of cancer therapy is always taken, among various therapies including radiofrequency ablation, arterial chemoembolization, etc.

When it comes to a hepatic cancer, there are cases in which other treatment methods than surgery, including radiofrequency ablation or arterial chemoembolization, should be taken into consideration, or which they should be combined with surgeries. In pursuit of an optimal treatment, the “hepato-biliary-pancreatic group” has been organized involving the Divisions of Diagnostic Radiology, Interventional Radiology and Gastrointestinal Oncology as well as the Radiation and Proton Therapy Center and this Division of Hepato-Biliary-Pancreatic Surgery. The group has a weekly medical conference, where all the images from examinations are reviewed for each one of the patients, and the best possible diagnosis and the optimal therapy strategy are discussed and decided from the broad viewpoints.

A Word from the Division Staff

We are often engaged in hours-long (sometimes 10 hours or longer) operations, because surgeries in the hepato-biliary-pancreatic region can often include such complicated procedures as hepatectomy or pancreatectomy. Also, behind cancers in this region, there are often other diseases like chronic hepatitis, hepatic cirrhosis and obstructive jaundice (jaundice with obstructed biliary duct) making the patients’ conditions even more complicated. We deal with such complicated cases, but still pursuit complete cure by all means, utilizing our own expertise and wisdom from other clinical divisions, and performing difficult surgeries.
