Large intestine, which follows small intestine in the digestive apparatus, is a 1.5 or 2-meter-long tube-like organ consisting of colon, rectum and anus, and cancer detected there is called “cancer of colon and rectum.” Sometimes it is called “colon cancer” or “rectum cancer” respectively, depending on where it is detected. The incidence rate for colon and rectum cancer starts increasing at fifties of age, and keeps increasing as the age number goes bigger. Also, the morbidity rate for the disease is the third biggest after prostate gland cancer and gastric cancer for male, and the second biggest after breast cancer for female (according to the data from the 2017 cancer morbidity survey based on the estimated figures for national cancer registration by region) in Japan. It proves how crucial it is to detect the disease at early stages and to provide optimal therapies. Colon and rectum cancers can be treated by endoscopic therapy, surgical therapy, systemic chemotherapy, radiation therapy and symptomatic therapy, or combinations of these therapies, in accordance with the tumor location and progression. Among them, the Division of Colon and Rectal Surgery handles “surgical therapy (operations).”
Even though colon and rectum cancers at early stages can be thoroughly removed by endoscopic procedure, which is to resect a cancer with an endoscope inserted through anus, the cases not applicable for endoscopic surgery as well as advanced cancers need open abdominal operations, laparoscopic surgeries or operations assisted by the robotic surgical system (the da Vinci Surgical System) at the Division of Colon and Rectal Surgery. The division at the SCC is known as one of the best medical institutions for colon and rectum cancer treatment in Japan, with around 720 surgeries a year being performed, including 550 for primary colon and rectum cancers.
At this division, more than 90% of colon and rectum cancer operations are done with laparoscopy (including procedures assisted by the robotic surgical system). Compared with open abdominal surgeries, laparoscopic procedures have advantages for patients, which include less blood loss, smaller operative scars, less postoperative pains, being far less invasive and earlier social rehabilitations.
Robotic surgery is a laparoscopic procedure assisted by the robotic surgical system. It offers such advantages as high possibilities of complete cure and conservation of void and sexual functions, in addition to those which a laparoscopic procedure offers conventionally. At the SCC, the state-of-art robotic surgical system called “da Vinci” was installed in December, 2011. So far, more than 1,300 robotic surgeries in total have been performed up until October, 2021, which is one of the biggest numbers in Japan, yielding so many excellent outcomes.
In the conventional surgical method for rectum cancer, where the anus is located close, it will be removed together with the tumor mass and replaced by a permanent stoma. At this division of the SCC, a part of anal muscle (i.e., external anal sphincter muscle) can be preserved when the cancer is removed. This anus preservation procedure, or the intersphincteric resection (ISR), is being performed for more than 90% of the patients with rectum cancer here, which proves our efforts for preserving anus as much as possible. At the same time, the employing rate for temporary stoma, which is often required as a part of anus preservation procedure, is quite low here and it’s one of the features of the therapy provided at this division.
For advanced cases, sometimes the decision will be made not to provide a surgical therapy when the cancer is found invading or metastasizing in the surrounding organs. However, we do not draw a line for a surgery, because it is the ultimate cancer therapy for complete cure of it. We always pursue the possibility for performing a surgery, including an extended procedure resecting the tumor mass as well as the surrounding organs where the tumor is located in.
Colon and rectum cancers are likely to lower the quality of life of the patients after the surgeries, as they naturally involve urination, bowel movement and sexual activity. Although the most crucial fact about a colon and rectum cancer procedure is that it should guarantee the complete removal the tumor mass, we always pursue ways to minimize the function losses while curing the disease skillfully. As we intend to confront it by teaming up with patients, we hope they will come to us without hesitation when they have any inquiries about the disease.
