Features of Plastic and Reconstructive Surgery

The Division of Plastic and Reconstructive Surgery specializes in “reconstructive surgery” which is to reclaim what is lost for the cancer treatment, including some body parts and functions as well as appearances. For surgical treatments of cancers, some parts of organs will have to be resected together with the tumor mass for the complete cure. When a cancer is small, the resected region can be small, too. However, on the contrary, when it needs to be resected to a large extent, it can affect appearance or physical functions and it might inconvenience the life after the treatment. At the Division of Plastic and Reconstructive Surgery, the affected regions are treated by “autogenic tissue transplantation (grafting)” which is transplanting the patient’s own tissues taken from other body regions. For example, if a whole breast is resected for a breast cancer surgery, the breast reconstruction will be performed. The breast reconstruction surgery is a typical procedure performed at this division. Nowadays such advanced reconstructive surgeries requiring very high skills as “free patch micrographic surgery” are being widely performed. “Free patch micrographic surgery” is a complex procedure transplanting whole tissues including skin, muscle, bone, etc. to the resected region by stitching very thin blood vessels with only 2 or 3 mm in diameter
together under a microscope (i.e., a procedure called microsurgery). Even for progressed cancers, which used to be considered as almost unresectable, total resections can be done nowadays without affecting physical functions or appearances much because of the skilled reconstructive surgeries being combined and customized for each case. The role of plastic and reconstructive surgery is getting more and more crucial in cancer treatment.

Features and Therapeutic Strategies of the Division at the SCC

We participate in cancer treatment in a planned manner starting prior to surgeries.

We always work together with other clinical departments in association with cancer resections, which include the Divisions of Head and Neck Surgery, Hepato-Biliary-Pancreatic Surgery, Dermatology, Orthopedic Oncology, Breast Surgery, Esophageal Surgery, Neurosurgery and Ophthalmology. Prior to the reconstructive surgeries that we perform, we put considerable thoughts to both appearances and physical functions including voice production, swallowing, locomotion as well as other body movements. In order to bring on favorable outcomes, we repeatedly discuss on the proposed extent of resection and the required reconstruction procedure with the surgeon in charge even during the planning phase. Our participation is always based on the planned manner being involved in the whole course of cancer treatment.

Body functions can be resumed much faster when proper post-operational care is provided.

We continue supporting our patients after the reconstructive surgeries through collaborations with the Divisions of Oral Surgery and Rehabilitation Medicine as well as the Dietary Department when they try to resume body functions and improve the qualities of their lives. We also work together with the Division of Rehabilitation Medicine when a patient has lymphatic edema, which is not unusual at all after the lymph node dissection or the radiation therapy focused on there because of the stagnation of lymph flow. When a patient starts having lymphatic edema, the pressure therapy will be provided at the Division of Rehabilitation Medicine in the first place. However, when it doesn’t work well to improve the lymphatic edema, a surgery to let the accumulated lymph fluid in the limbs flow into the vein by sewing the lymph vessel, which is less than 1mm in diameter, to the vein at the Division of Plastic and Reconstructive Surgery. This delicate procedure, called lymphatic venular anastomosis (LVA), requires not just advanced and sensitive surgical techniques but also the team medicine providing the patient with wide-ranged supports. The team LVA consisting of the Divisions of Plastic and Reconstructive Surgery and Rehabilitation Medicine, as well as the Dietary Department and the ward nurses get together once a month as for the multidisciplinary conference to share information.

A Word from the Division Staff

We employ such advanced skills as autogenic tissue transplantation as well as microsurgery to support cancer resections working together with other clinical division handling surgeries, when they try to save as much physical functions or appearances as possible. We also promote surgeries to improve the quality of life after the cancer treatment through breast reconstruction or lymphatic edema treatment. It’s part of our thorough efforts to support a patient letting him/her live a normal life after the cancer treatment. However, the cancer resection itself is not within our field. Please go to the Division of Dermatology for skin cancer, and to the Division of Orthopedic Oncology for bone and soft tissue tumor.
